Set up a care assessment and let us get to know you. Our Director of Clinical Care meets with each prospective client and their loved ones to assess both medical and psychosocial needs. During these detailed discussions, our team will begin to construct a comprehensive, fully individualized plan of care.
Following the assessment, our Director of Clinical Care will work closely with the Legacy team to select a Care Companion (or a team of Care Companions) who best meets the client’s needs and caregiving preferences. We individually match each of our caregivers to our clients based on their clinical skills and caregiving styles.
